I read on another forum when I was googling a solution to this problem, that you could avoid this error by holding onto the disk when you put it into the machine and give it a bit of resistance to pull against before letting go and letting it feed into the xbox. Sounded daft, but it works.

I won't categorise this as an answer, because obviously MS has got something wrong here and hopefully they will come up with a proper solution.
