Start a game with Palm
You must be playing in Score Attack or Normal Play, the Training mode doesn't unlock achievements. Default settings are required for achievements to unlock in Normal Play. No restarts.
Good news for people more interested in gamerscore than learning to play, most of the achievements are doable in Novice except as otherwise noted. For those of you who were lucky enough to receive the DLC card with your order, note that achievements do not unlock in Ver 1.01.

Defeat Spiritual Larsa, the True Last Boss. In order to even fight her, you must reach the end of God Mode without losing even a single life. Have fun, good luck.
All of these achievements have to be done in Black Label Arcade or XBox360 modes, of course. Standard conditions apply i.e. no restarts, default settings, and no unlocks in practice mode.
